Financial Obligation: I have read my ROGace & South Towns Tennis & Pickleball Club’s Policies, Rules and Procedures. I understand that House Charging privileges are a benefit of membership. For house charges not paid in full, a 2% finance charge will be assessed on outstanding balances on the last day of each month. To avoid finance charges, please pay your balances in full each month by check, cash or EFT.

Parent/Guardian Release: I hereby give permission for my child(ren) to participate in any and all activities of ROGace & South Towns Tennis & Pickleball Club programs. I understand that ROGace, South Towns Tennis & Pickleball Club, its staff or independent contractors is/are not responsible for any illness, injuries or harm incurred by my child’s involvement in this sport. All participants must be 18 years or older or must have a parent’s signature. You should always consult a doctor before participating in any sports activity. From time to time, food may be served or purchased at the club, which may contain allergens, including but not limited to peanuts and dairy products. Please be aware and take the necessary precautions. In addition, ROGace and South Towns Tennis & Pickleball Club is not responsible for the administration or the assistance in the administration of any drug, medication or medical device, whether prescription or over the counter, to or for any member, person or guest regardless of age or capacity. If you have any allergies, you, a parent or guardian are solely responsible for your medical condition and the administration of any required drug or medication.

Make-up classes will only be scheduled during the last week of the session. Each player will be eligible for one make-up date for each class that they are signed up for in a session. For example, if a player is signed up for one class, they will be eligible to participate in one make-up class, and if a player is signed up for two classes in a given session, they’re eligible for two make-up classes. Players do not need to report their absence, but it would be kindly appreciated. During the 7th week of a session, ROGace will send out the weekly schedule for available make-up classes.

Refunds for missed classes or withdrawing from a session will only be granted to players with documented long-term (3+ weeks) medical-related reasons (make-ups are available for the classes missed due to cold etc.). Scheduling conflicts, vacations, inclement weather cancellations, etc. do not qualify for refunds. Approved refunds will be prorated and made in the form of an account credit that may be applied towards a future session.

(OPTIONAL). By initialing here, I am providing permission to ROGace and South Towns Tennis & Pickleball Club to use my child’s likeness, image or video clip of my child in promotional and training materials. These materials may include website postings, social media postings, social media advertisements, posters and printed promotional materials. Your child’s personal information, such as last name, age, school attended, or address, will never be included with their photo or likeness. Please note that by refusing to initial, you are opting-out of this permission and this will not have any effect on your registration or enrollment in ROGace’s program.
